Referral Requirements

All appointments require a valid referral from a medical practitioner. 

• General Practitioner referrals are valid for 12 months. A new referral is required for ongoing care. 
• Specialist referrals (e.g., obstetrician, paediatrician) are valid for 3 months.

What to Bring to Your Appointment

To ensure the best care for your child, please bring:

• Child Health Recored Book

• Relevent reports or letters (e.g., school, medical, psychology, speech therapy, audiology, or imaging results)

• A current list of medications, including dosages and strengths
